What You Will Do
  • Complete UAS NY assessments during in home visits or via telehealth
  • Review medical documentation and record accurate detailed findings
  • Educate consumers and communicate with providers when needed
  • Maintain timely high quality documentation that meets all NYS standards
  • Manage your schedule route planning and daily workflow
  • Follow all conflict free requirements under the NYS Department of Health
Qualifications We Look For What You Need

Must have own car for daily travel between assessment locations

Public transportation and ridesharing are not permitted

  • 1โ€“2 yearsof experience as an RNin one-on-one in-home, community, skilled nursing facility, long term and geriatric care required
  • RN license valid inNew York
  • BSNis required
  • Familiarity withUniform Assessment System (UAS)-NYis a plus
  • Strong communication, time management, and Electronic Health Record (EHR) skills
  • Reliable transportation for in-person visits
Minimum Qualifications
  • Active RN license
  • At least 22 months of home health care experience within the last 15 years, includingone or moreof the following:
    • RN/LPN experience with a CHHA, LHCSA, or MLTCP
    • RN Care Manager for home- or community-based services (Hospice/Assisted Living included)
    • NP providing primary care to homebound individuals
    • HHA or Personal Assistant experience in certified/licensed home care settings

Must meet conflict-free requirements under NYS DOH.

Preferred
  • Recent experience completing OASIS UAS CHA or pediatric assessments
  • Experience managing chronic conditions in older adults
  • Combination of Home Health Aide experience and private duty RN or LPN work
  • Familiarity with the Uniform Assessment System for New York
Additional Backgrounds Also Considered
  • RN Case Manager experience in hospitals or health plans
  • Experience with telephonic only assessments
  • RN work in group home or residential settings
