As an integral member of our mental health team, your responsibilities will include:

• Conducting comprehensive clinical assessments for patients referred to hospice services, ensuring that individualized treatment plans are tailored to each patient’s unique psychosocial needs.
• Utilizing and implementing evidence-based therapeutic modalities such as cognitive-behavioral therapy (CBT), narrative therapy, and solution-focused therapy to address the mental health challenges faced by patients.
• Maintaining accurate and timely clinical documentation compliant with HIPAA regulations, ensuring confidentiality and adherence to best practices in patient care.
• Engaging in interdisciplinary collaboration with physicians, nurses, and other mental health professionals to develop holistic care approaches that integrate medical and psychological treatment.
• Participating in regular case consultations and peer review meetings to enhance service delivery and clinical practice, fostering a culture of continuous learning.
• Providing education and guidance to patients and their families regarding mental health concepts, treatment options, and available community resources for additional support.
• Staying informed about advancements in mental health practices and incorporating new knowledge into your clinical work to ensure the highest standard of care.

You will primarily work with patients enrolled in hospice care, many of whom are experiencing complex emotional and psychological challenges related to terminal illness. Your role will involve supporting both patients and their families through difficult conversations, grief counseling, and strategies to cope with the impending transitions they face. Your compassionate approach will be vital in helping them find peace and understanding during this sensitive time.

To be successful in this role, candidates must possess the following qualifications:

• A Master’s degree in Social Work from an accredited institution.
• A valid LCSW license to practice in the state of Connecticut.
• At least 3 years of direct clinical experience, specifically in providing psychotherapy services.
• Proven expertise in evidence-based therapeutic interventions and a deep understanding of mental health issues prevalent in hospice patients.
• Exceptional communication and interpersonal skills to foster trust and rapport with patients and their families.

Preference will be given to candidates with experience in hospice care, palliative medicine, or trauma-informed care practices. Demonstrated abilities to apply solution-focused interventions effectively within a multi-disciplinary team environment will be highly regarded.

We offer a competitive salary range of USD 80,000 – 100,000 annually, commensurate with experience and qualifications.
