• Conduct thorough clinical assessments to evaluate patients’ psychosocial needs, ensuring a comprehensive understanding of their circumstances.
• Develop and implement tailored treatment plans that employ evidence-based therapeutic interventions, such as cognitive-behavioral therapy and solution-focused therapy.
• Maintain accurate clinical documentation, ensuring compliance with all regulatory requirements and organizational standards, while minimizing administrative burdens.
• Collaborate with a skilled multidisciplinary treatment team—including physicians, nurses, and palliative caregivers—to address the holistic needs of our patients.
• Facilitate family meetings to provide education and emotional support, equipping families with the tools needed to navigate end-of-life scenarios.
• Stay abreast of the latest research and methodologies in mental health to enhance service delivery and patient care.

Your role will primarily focus on patients and families receiving hospice care, who face significant emotional and psychological challenges. You will be instrumental in facilitating coping strategies and emotional support, allowing patients and loved ones to navigate their journey with dignity and grace. • A Master’s Degree in Social Work (MSW) from an accredited institution is required.
• A valid Licensed Clinical Social Worker (LCSW) license in the state of Connecticut is essential to perform effectively in this role.
