Remote Mental Health Case Manager, Mercy Virtual Collaborative Care
Join a virtual collaborative care team where learning, evidence-based practice and consistent communication drive better outcomes. This role will provide case management/therapy for patients and families to enhance their psychosocial functioning.
  • Ensures safety, confidentiality, regulatory compliance, and adherence to hospital and infection control standards.
  • Coordinates interdisciplinary care, advocates for patients and families, and oversees safe, comprehensive discharge planning.
  • Provides behavioral health case management, assessments, therapy, crisis intervention, and ongoing treatment planning with accurate documentation.
  • Communicates effectively with patients, families, and community partners while maintaining strong clinical, organizational, and technology skills.

Qualifications:
  • Education: Master's degree required in Social Work, Counseling or another human-service related field.
  • Licensure: Licensed Clinical Social Worker (LCSW), Licensed Master Social Worker (LMSW), or Licensed Professional Counselor (LPC) in Missouri required. Only a Licensed Clinical Social Worker (LCSW) is accepted at Mercy Hospital Perry.
  • Experience: Minimum 2-3 years Behavioral Health experience preferred.
  • Preferred Other: Co-worker must be able to communicate orally, in writing and demonstrate proficient computer skills. Co-worker must be able to listen and to understand and explain complex situations.
