Position Details:
Remote Mental Health Case Manager (LCSW) - Mercy Virtual Collaborative Care
Make a meaningful impact from anywhere. Join Mercy's Virtual Collaborative Care team and help patients and families navigate behavioral health challenges through compassionate, evidence-based care. As a Remote Mental Health Case Manager, you will provide psychosocial assessments, therapy, crisis intervention, care coordination, and discharge planning while partnering with an interdisciplinary team to improve patient outcomes.
What You'll Do
  • Conduct behavioral health assessments and develop individualized treatment plans.
  • Provide individual, family, and crisis intervention services.
  • Coordinate care with physicians, nursing staff, therapists, and community resources.
  • Support patients and families with education, resource navigation, and discharge planning.
  • Advocate for patients and encourage active participation in treatment goals.
  • Monitor patient progress and adjust care plans as needed.
  • Maintain accurate and timely clinical documentation in Epic.
  • Collaborate with internal and external partners to ensure continuity of care.
Qualifications
  • Master's degree in Social Work, Counseling, or a related human services field required.
  • Licensed Clinical Social Worker (LCSW) required.
  • 2-3 years of behavioral health experience preferred.
  • Strong assessment, communication, crisis intervention, and care coordination skills.
  • Proficiency with Microsoft Office and electronic medical records.

  • Monday-Friday, 8:00 AM - 4:30 PM
  • No weekends or on-call responsibilities
