About Us
Strive Health provides on-site psychiatric and behavioral health services to Skilled Nursing Facilities and Assisted Living communities throughout Wisconsin and Illinois. Our team partners closely with facility staff to improve behavioral health outcomes, support psychotropic stewardship, enhance resident quality of life, and provide consistent, compassionate psychiatric care.
We are seeking a highly organized and compassionate Behavioral Health Care Coordinator to support our growing Behavioral Health Integration (BHI) and care management operations.
Position Summary
The Behavioral Health Care Coordinator supports residents receiving ongoing psychiatric and behavioral health services through care coordination, patient outreach, interdisciplinary communication, documentation support, and behavioral health management workflows.
This role works closely with psychiatric nurse practitioners, therapists, nursing staff, social services, families, pharmacies, and other healthcare professionals to support continuity of care and improve behavioral health outcomes across long term care settings.
This is an operations-focused behavioral healthcare role ideal for candidates with experience in:
• BHI
• CCM
• RPM
• Skilled nursing
• Behavioral health
• Care coordination
• Long term care operations
Responsibilities
• Coordinate ongoing behavioral health management services for residents enrolled in BHI and related care management programs
• Conduct patient and facility outreach to support continuity of care, follow up, and care coordination activities
• Collaborate with psychiatric providers, therapists, nursing staff, social services, pharmacies, and families regarding resident behavioral health needs
• Monitor and document behavioral symptoms, medication response, follow up needs, and care coordination activities
• Assist with behavioral health care plan maintenance and monthly care management workflows
• Track and document cumulative monthly time and qualifying care management activities in accordance with Medicare guidelines
• Obtain and document patient or responsible party consents for participation in care management programs
• Escalate clinical concerns, behavioral changes, medication issues, or urgent resident needs to providers appropriately
• Support psychotropic stewardship and GDR monitoring workflows in collaboration with psychiatric providers and facility teams
• Maintain accurate documentation within the EMR and care management platforms
• Assist with patient eligibility verification, insurance coordination, and workflow tracking
• Participate in interdisciplinary collaboration and operational process improvement initiatives
Minimum Qualifications
• Minimum 2 years of healthcare experience required
• Experience in behavioral health, skilled nursing, long term care, care coordination, chronic care management, or related healthcare settings strongly preferred
• Strong communication, organizational, and documentation skills
• Ability to manage high-volume workflows in a fast-paced healthcare environment
• Comfortable communicating with interdisciplinary healthcare teams, residents, families, and facility staff
Preferred Qualifications
Preferred but not required:
• LCSW
• LPC/LCPC
• RN
• Medical Assistant certification
• Behavioral health or care coordination certifications
Experience with any of the following is strongly preferred:
• BHI
• CCM
• RPM
• TCM
• PCM
• Medicare care management programs
• Skilled Nursing Facilities or Assisted Living communities
• Behavioral health or psychiatric populations
• Psychotropic medication monitoring
• Care management documentation workflows
