Overview
Chronic Care Staffing is seeking a compassionate and detail-oriented Remote Care Coordinator - CMA/RMA to support patients managing chronic conditions through proactive outreach, education, and care coordination. In this fully remote role, you will serve as a trusted point of contact for patients, helping them stay engaged in their care plans while supporting providers through accurate documentation and communication.
This opportunity is ideal for certified medical professionals who enjoy patient interaction, independent work, and improving long-term patient outcomes in a remote healthcare environment.
Responsibilities
  • Conduct structured monthly outreach calls to monitor patient progress and engagement.
  • Verify medication adherence and assist with health risk assessments.
  • Support Chronic Care Management (CCM), Transitional Care Management (TCM), and Remote Patient Monitoring (RPM) initiatives.
  • Document patient interactions, care updates, and clinical information accurately within EHR systems.
  • Coordinate communication between patients, families, providers, and internal care teams.
  • Identify barriers to care and escalate patient concerns when necessary.
  • Educate patients on care plans, treatment goals, and available support resources.
  • Maintain HIPAA compliance and CMS documentation standards at all times.
Requirements
Must Have
  • Residency within the PST or MST time zone.
  • Active, unencumbered CMA or RMA certification.
  • Current Basic Life Support (BLS) certification.
  • Recent experience in a clinical, care coordination, or chronic care management setting.
  • Strong proficiency with EHR systems and Google Suite.
  • Dedicated, HIPAA-compliant home office with reliable high-speed internet.
  • Excellent communication, organization, and patient engagement skills.
Nice to Have
  • Previous experience with CCM, TCM, or RPM programs.
  • Experience navigating community resources for patient support.
  • Prior success working in independent remote or virtual healthcare environments.
