Key Responsibilities- Conduct thorough assessments and develop individualized treatment plans tailored to the unique needs of each client.
- Provide direct counseling services through virtual platforms, utilizing evidence-based practices to ensure the highest quality of care.
- Collaborate with team members to create and implement effective crisis plans, positive behavior support strategies, and medication management guidelines.
- Complete administrative documentation promptly, ensuring compliance with regulatory standards and maintaining client confidentiality.
- Actively engage in licensing and accreditation processes, demonstrating adherence to established professional standards.
- Develop and maintain strong communication with state agencies and community partners to enhance service delivery and expand resources available to clients.
- Participate in ongoing professional development activities, including attending relevant conferences and trainings.
- Promote and embody the mission and values of Grace Hospital both within the team and in the community at large.
- Perform additional duties as directed by the Clinic Director to meet the evolving needs of our clients and organization.
The LMHC will work with diverse patient populations, including individuals seeking therapeutic support for emotional challenges, substance use disorders, and those with developmental disabilities or acquired brain injuries. Each client’s background, experiences, and needs are meticulously considered in developing a supportive therapeutic relationship and effective intervention plans that align with their goals.
Qualifications- A Master's Degree in a behavioral health field is required, ensuring a solid foundation of clinical knowledge.
- Candidates must hold an independent LMHC license recognized by the state of Connecticut.
- Proficiency in maintaining clinical records and producing relevant reports in accordance with best practices.
- Strong interpersonal skills and the ability to work collaboratively within interdisciplinary teams are essential.
- Effective communication skills, both verbal and written, are critical for successful interactions with clients and other professionals.
- Demonstrated ability to exercise professional judgment and make sound decisions based on clinical expertise and patient welfare.
- Experience in telehealth services is advantageous, as our counseling primarily occurs through remote modalities.
- Previous work experience in community behavioral health settings will be highly regarded.
- Familiarity with evidence-based screening tools and therapeutic modalities that support diverse client needs is a plus.
- Proven history of maintaining rapport with community agencies and stakeholders to enhance client care.
