• Collaborate with primary care providers to identify, treat, and manage mental health and psychosocial issues, thereby functioning as an integral part of the patient care team.
• Conduct comprehensive patient intakes, focusing on diagnostic assessments and functional evaluations, and recommend treatment goals and plans to primary care providers.
• Deliver brief, focused mental health interventions for patients in need, tailoring approaches to individual treatment plans.
• Guide primary care providers on referrals to specialized mental health facilities when patient needs exceed standard clinic capabilities.
• Initiate follow-up communications to evaluate patient progress and effectiveness of treatment strategies, adapting approaches as necessary.
• Develop and implement relapse prevention plans to support patients in maintaining stable functioning, focusing on sustainable mental health practices.
• Monitor patient health services delivery related to behavioral health care, coordinating with external treatment providers with patient consent.
• Document patient progress thoroughly in treatment charts, ensuring compliance with ethical and legal standards.
• Keep primary care teams informed about patient needs and treatment progression, fostering collaborative treatment planning.
• Empower patients by equipping them with self-management skills and educational resources, fostering independence in their recovery journey.
• Provide insights and consultations to clinic management regarding behavioral health services and program evaluation efforts.
• Support patients in adhering to medical treatments recommended by primary care providers, including strategies to manage medication-related side effects.
• Adhere to the Code of Ethical Conduct for Clinical Professionals and complete other assigned duties to support clinic operations.

This role primarily serves a diverse patient population, encompassing a range of ages and backgrounds. The ability to assess individual needs and accommodate various cultural contexts is vital in providing effective care.

• A Master’s Degree in Clinical Counseling or a related field is required.
• Valid Licensed Professional Counselor (LPC) license is essential.
• Strong understanding of mental health assessment, treatment planning, developmental psychology, and therapeutic modalities.
• Thorough knowledge of community resources, psychodiagnostics, and psychotherapeutic techniques.
• Ability to build and maintain effective relationships with patients, their families, and other professionals.
• Strong organizational skills, with the ability to maintain accurate clinical records and prepare reports.
• Excellent communication skills for expressing ideas clearly and effectively.
• Sound judgment and decision-making abilities grounded in professional training.

• Experience working in a community health setting and familiarity with Medicaid and Medicare guidelines is preferred.
• Bilingual proficiency in English and Spanish is highly regarded but not mandatory.
• Previous experience in team-based care models is advantageous.

The position offers a competitive salary range of USD 87,000 – 105,000 annually, reflective of experience and qualifications.
