• Collaborate with primary care practitioners to identify, assess, and manage mental health and psychosocial challenges, enhancing the overall efficacy of the primary care team.
• Conduct thorough patient intakes focused on diagnostic and functional evaluations, aiding in the creation of personalized therapy objectives and treatment recommendations.
• Provide timely and focused mental health interventions for patients requiring emergency support, ensuring immediate care is available.
• Advise primary care professionals on the appropriate mental health services, making referrals to specialized practitioners when necessary.
• Facilitate regular follow-up evaluations to monitor patient progress, adapt treatment plans as needed, and document all findings in comprehensive patient records.
• Develop and implement relapse prevention strategies tailored to each patient's needs, empowering them with tools for sustained recovery.
• Identify patients at risk of declining health and participate in formulating preventative programs that address their unique requirements.
• Document patient progress, maintaining complete and accurate treatment records, and continuously communicate with primary care providers regarding patient status.
• Utilize evidence-based methodologies to promote behavioral changes that support physical and mental health improvements among patients.
• Deliver educational resources and self-management techniques that empower patients to take an active role in their recovery journey.
• Consult with clinic management and interdisciplinary team members to improve behavioral health services and contribute to program development initiatives.
• Uphold the Clinical Professionals' Code of Ethics, ensuring that all patient interactions adhere to the highest professional standards.
• Perform additional duties as assigned to support the overall mission of Brightways Recovery Plus, Inc.

Your work will involve engaging with a diverse population of patients facing various mental health and psychosocial challenges, ranging from anxiety and depression to stress-related disorders. Our clients come from various backgrounds and age groups, requiring a compassionate and culturally competent approach in addressing their unique needs.

• A Master’s Degree in Clinical Counseling or a related field is required.
• Current and valid LPC (Licensed Professional Counselor) license.
• Proficient understanding of mental health evaluation, treatment planning, family therapy, and various therapeutic methodologies.
• Experience with DSM-IV diagnoses and behavioral therapy principles is essential.
• Strong therapeutic communication skills, with fluency in both English and Spanish preferred.
• Ability to analyze complex patient needs and provide tailored treatment interventions.
• Adept at utilizing telehealth technologies to facilitate effective client interactions.
• Personal resilience and the mental/physical health capacity to manage the demands of a remote counseling role.

• Demonstrated experience in a primary care or clinical counseling setting.
• Familiarity with community resources and the ability to navigate social support systems beneficial to patient care.
• Experience developing preventative mental health programs is a plus.
• Prior involvement in multidisciplinary teams that integrate behavioral health with primary care is highly desirable.
