CareHarmony's Intake Coordinators (LPN) (NLC) (LVN) work comprehensively with providers to deliver value-based care management initiatives for their patients.
CareHarmony is seeking an experienced Licensed Practical Nurse - LPN Nurse (LPN) (NLC) (LVN) with at least 3 years of direct patient-facing work experience; that thrives in a fast-paced environment, is self-motivated, has impeccable attention to detail, and values the impact they can have on a patient's healthcare journey.
You will have experience identifying resources and coordinating needs for chronic care management patients. This role is great for anyone who loves meeting and assisting new patients daily.

Requirements
Responsibilities:
  • Accepting transfers from the Patient Enrollment team to conduct preliminary health assessments for newly enrolled patients in our network.
  • Resolve patients' questions and create an open dialogue to understand needs.
  • Identify and coordinate community resources with patients that would benefit their care.
  • Provide patient education and health literacy on the management of chronic conditions.
  • Assist with medication management, including identifying potential medication concerns, adherence, and coordinating refills.
  • Assist in ensuring timely delivery of services to your patients; Home Health, DME, Home Infusion, and other critical needs.
  • This individual must be able to quickly adapt to a fast-paced work environment. This role requires most of your shift on the phone.

Additional Requirements:
  • Active Compact/Multi-State license (LPN) (LVN)
  • Technical aptitude - Microsoft Office Suite
  • Excellent written and verbal communication skills

Remote Requirements:
  • Must have active high-speed Wi-Fi
  • Must have a home office or HIPAA-compliant workspace
