Job description
At NorthBay Health, we are seeking a Licensed Clinical Social Worker (LCSW) or experienced MSW to join our team in Fairfield, CA. This full-time, weekday position supports both inpatient units and Emergency Department mental health cases, with planned weekend/holiday coverage as needed. This role is ideal for a strong clinician passionate about behavioral health, care coordination, and supporting patients with complex psychosocial needs across the continuum of care. (Please note: supervision for LCSW hours is not available at this time.)
Responsibilities:
- Provide direct clinical social work services, including counseling, discharge planning, and case management for hospital patients and their families
- Perform comprehensive biopsychosocial assessments addressing emotional, psychological, social, financial, environmental, and follow-up referral needs
- Support Emergency Department mental health cases, including safety planning and intervention (training available for 5150 certification if needed)
- Initiate and discontinue 5150 involuntary psychiatric holds within NorthBay Medical Center, VacaValley Emergency Department, and inpatient units in accordance with Welfare and Institutions Code Section 5150
- Manage psychosocially complex patients across the continuum of care by developing care plans, monitoring progress toward goals, and addressing barriers to length of stay and discharge
- Collaborate closely with physicians, nurses, case managers, and interdisciplinary healthcare teams to ensure coordinated, patient-centered care
- Facilitate integration with community-based resources and partner with external agencies to optimize patient health outcomes
- Communicate key patient information effectively across the continuum of care to support continuity and safe transitions
- Document all assessments, interventions, and care plans accurately in the electronic medical record
- Ensure all activities align with NorthBay Health's mission, values, policies, procedures, and regulatory standards

Compensation Structure
NorthBay Health uses a structured compensation framework. Staff-level positions use a step-based system (Steps 1-5) based on years of directly related experience, with Step 5 representing 20 or more years of experience in the role. Manager level and above positions are paid a fixed annual base salary and are eligible for a variable incentive compensation plan. Physician compensation is structured based on specialty and role requirements.

Education:
- Master of Social Work from a school of Social Work accredited by the Council on Social Work Education.
Licensure:
- California BBS Licensed Clinical Social Worker (LCSW), *CCS paneling required if working with the pediatric/Neonatal Intensive Care (NICU) population.
Experience:
- Minimum of two years' experience within the acute health care setting or the community health setting
Skills:
- Strong clinical assessment skills.

- Knowledge of child, elder and dependent adult abuse and domestic violence reporting requirements and other significant legislation and laws regulating social work practice, e.g. Tarasoff, HIPAA, etc. Ability to make appropriate interventions based on law, professional ethics and policy.
- Knowledge of suicidal behavior, the ability to assess lethality, and the ability to develop appropriate treatment plan. Knowledge of an ability to effectively use DSM V. Knowledge of legal requirements and guidelines for providing psychiatric services including the appropriate use of 5150.
- Ability to successfully manage conflict.
- Basic proficiency with word processing required.
- Experience with Cerner, and Power Point preferred.
- Knowledge of acute care, subacute care, long-term care, home care, hospice interventions, rehabilitation options, County mental health services, and other community resources and requirements- strongly preferred.
- Ability to conceptualize clinical situations and demonstrate application of theory in case management and psychotherapeutic responsibilities
