• Conduct comprehensive psychosocial assessments to understand clients’ mental health concerns, strengths, and resources, guiding effective intervention strategies.
• Provide individual, family, and group therapy sessions utilizing evidence-based treatment modalities tailored to the needs of patients facing end-of-life issues.
• Implement therapeutic practices such as cognitive-behavioral therapy (CBT), solution-focused therapy, and trauma-informed care to support the mental and emotional well-being of clients and their families.
• Develop and execute personalized treatment plans, establishing clear goals and actionable interventions to foster client progression.
• Collaborate with a multidisciplinary team, including psychiatrists, psychologists, and nursing staff, to ensure continuum of care and coordinated treatment approaches.
• Engage in crisis intervention, conducting risk assessments and safety planning when necessary to ensure client safety and emotional stability.
• Maintain detailed, accurate, and timely clinical documentation that adheres to ethical and legal standards, fostering accountability and quality care.
• Facilitate group therapy sessions, psychoeducational classes, and patient support groups, enhancing community support and education focused on mental health and grief.

The primary focus of your role will be to support clients facing chronic illness and life-limiting conditions, along with their families. You will work with diverse individuals, offering an empathetic presence during critical transitions, whether navigating grief, depression, or anxiety related to end-of-life planning. Your expertise will greatly enhance the hospice experience for both patients and their loved ones, promoting resilience and coping strategies during emotionally challenging times.

• A Master’s degree in Social Work from an accredited program.
• Active state licensure as a Licensed Clinical Social Worker (LCSW).
• A minimum of two years of clinical experience, particularly within mental health or outpatient settings.
• Strong foundation in various evidence-based therapeutic modalities, with a focus on those suited for hospice and palliative care.
• Excellent interpersonal, communication, and assessment skills, with an ability to build trust and rapport with clients and families.

• Previous experience providing therapy in hospice or palliative care settings.
• Demonstrated understanding of grief and bereavement processes, particularly relating to end-of-life issues.
• Familiarity with remote therapy tools and platforms that facilitate online client interactions.
