You will collaborate virtually with a team of dedicated healthcare professionals, including psychiatrists, psychologists, and fellow social workers, to provide a cohesive approach to client care. • Conduct comprehensive psychosocial assessments to thoroughly understand each client’s mental health challenges, strengths, and available resources.
• Provide individual, family, and group therapy, utilizing evidence-based practices such as cognitive-behavioral therapy (CBT), solution-focused therapy, and trauma-focused therapy tailored to the unique needs of hospice patients.
• Design and implement individualized treatment plans with clear, measurable goals and interventions in collaboration with clients and their families.
• Facilitate supportive group therapy sessions and psychoeducation classes aimed at empowering clients and fostering community engagement.
• Engage in crisis intervention and risk assessment, ensuring client safety and addressing immediate mental health concerns effectively.
• Maintain accurate and timely documentation in accordance with ethical and legal standards, reflecting treatment progress and pertinent client information.
• Work alongside a treatment team to coordinate care, integrate services, and hold weekly interdisciplinary meetings to discuss client progress and adjust treatment plans as needed.
• Participate in ongoing training and development opportunities to remain current with best practices and enhance clinical competencies.

As a Licensed Clinical Social Worker at our hospice center, you will work with a diverse array of clients facing end-of-life issues, mental health disorders, and emotional distress. This role emphasizes empathy and connection, as you help individuals and families navigate grief, anxiety, and the challenges of facing serious health conditions while promoting resilience and understanding.

• Master's degree in Social Work from an accredited program.
• Active state licensure as a Licensed Clinical Social Worker (LCSW).
• A minimum of two years of experience in mental health or outpatient settings, preferably with hospice or palliative care settings.
• Strong competence in evidence-based therapeutic modalities along with excellent interpersonal and communication skills.
• Ability to perform complex assessments and develop comprehensive treatment plans that address both immediate and long-term needs of clients.

• Prior experience working in a hospice or end-of-life care environment.
• Familiarity with interdisciplinary team collaboration in a healthcare setting.
• Additional certifications or training in specific therapeutic modalities relevant to this setting.

The salary for this position ranges from USD 80,000 to 100,000 per year, commensurate with experience, plus a generous sign-on bonus.
