This role offers a flexible remote work setting, allowing you to engage with patients and families using telehealth technologies, including secure video conferencing, phone consultations, and digital documentation tools. • Conduct thorough assessments of patients and their families to understand their emotional, psychological, and social needs.
• Develop and implement individualized treatment plans that focus on coping strategies, grief support, and resource navigation.
• Deliver solution-focused therapy sessions, both virtually and via phone, to address issues related to trauma, anxiety, and depression.
• Maintain meticulous, confidential records in accordance with healthcare regulations and organizational policies.
• Collaborate with an interdisciplinary treatment team to coordinate care plans and enhance patient outcomes.
• Stay abreast of advancements in social work practices and hospice care to ensure the highest level of service delivery.

• A Bachelor’s degree in Social Work from an accredited institution is required; a Master’s degree is preferred.
• Possession of an active LCSW license in Colorado.

• Strong organizational skills to effectively manage caseloads and documentation.

A minimum of two years of experience in social work, particularly within healthcare settings, is required.
