Position Purpose
Healthcare Navigators connect Veterans with VA health care benefits and community health care services. They provide case management, care coordination, health education, interdisciplinary collaboration, consultation, and administrative support. Healthcare Navigators work closely with each Veteran's primary care provider and assigned interdisciplinary treatment team. This position serves as the primary liaison between HAV (Homes for All Veterans) and VA or community health care systems. Healthcare Navigators support Veterans with complex needs who require assistance accessing health care services or following health care plans. They collaborate with assigned multidisciplinary teams, including medical, nursing, administrative, and case management personnel, to address these needs and provide timely, appropriate, equitable, and Veteran-centered care.
The Healthcare Navigator serves Veterans across the Denver Metro area and Northern Colorado, including Boulder, Longmont, and Greeley, through a combination of home-based, community-based, and office-based services. This highly collaborative role requires regular travel to meet clients where they are, fostering meaningful connections and supporting access to healthcare and related resources. This position generally operates during standard business hours, Monday through Friday, with the flexibility to work occasional evenings and weekends to effectively meet client and program needs.
Essential Duties
  • Manage day-to-day responsibilities in a remote work setting.
  • Conduct non-clinical assessments in collaboration with health care providers, Veterans, family members, and other significant supports.
  • Work with Veterans and their family members to develop care plans, monitor progress, and share updates with health care professionals as needed.
  • Provide comprehensive case management and care coordination and act as a health coach by proactively supporting Veterans in optimizing treatment interventions and outcomes.
  • Identify Veteran and family health education needs and provide education, services, and materials that match the Veteran's health literacy level.
  • Participate in regular supervision as determined by the supervisor and employee.
  • Regularly consult and collaborate with HAV and RMHS team members to address Veteran needs.
  • Participate in ongoing learning related to health care navigation and the Veteran population.
  • Develop collaborative working relationships with community health care providers, the VA health care system, and other referral networks.
  • Identify and report team activities or issues to the Supervisor and/or Director when they may positively or negatively affect service delivery under the contract.
  • Complete all required documentation, including client contacts, collateral contacts, case management interventions, health care assessments and care plans, behavioral health care visits, and physical health care visits.

Knowledge, Skills and Abilities
  • Communicate effectively with team members, colleagues, community partners, VA staff, and community health care staff.
  • Maintain strict confidentiality and comply with HIPAA requirements.
  • Maintain appropriate boundaries and a professional demeanor.
  • Work independently with minimal supervision.
  • Complete data entry and reporting as needed for contract requirements.
  • Meet required timelines for all documentation.
  • Demonstrate knowledge of VA benefits, crisis services, Medicaid, Medicare, and other community resources.
  • Use person-centered and trauma-informed approaches to care coordination services.
  • Adhere to RMHS values.
  • Knowledge and understanding of common symptoms of severe and persistent mental illness

Essential Functions
  • Attends staff, team and department meetings.
  • Attends in-services, staffing and other meetings with supervisor's approval. May be appointed to committees.
  • Participates in agency and community planning and education.
  • Develops and maintains records, plans and reports.
  • Lift and/or carry 20 lbs.
  • Sit, stand and walk for reasonable periods of time.
  • Maintains prompt and regular attendance.
  • Performs related work as assigned.
  • Statewide travel may be required. Travel expected 50% of the time.

Minimum Qualifications
  • Master's degree in counseling, social work, or a related field; or an equivalent combination of education and experience in health care or a related field.
  • Ability to complete training required by the VA and RMHS.
  • Ability to pass required background checks.

Preferred Qualifications
  • Experience working with Veterans, especially Veterans experiencing homelessness.
  • Working knowledge of, or professional or personal experience with, the VA or community health care system.
  • Two to five years of relevant experience, specifically in case management and/or care coordination in a health care environment.

Driving Requirements
  • Valid driver's license
  • Proof of motor vehicle insurance
  • No major violations within the past three years
  • No more than two moving violations within the past three years
  • Ability to meet and maintain agency driving requirements and operate agency vehicles
  • Drivers must upload proof of current auto insurance into Paycom/Relias every six months.
