This full-time remote position focuses on providing exceptional social work services to seniors in a hospice care setting. Key Responsibilities

As a Licensed Master Social Worker at Optimum Care Counseling, you will:

  • Conduct comprehensive assessments of clients' psychosocial needs, considering their emotional, mental, and social circumstances.
  • Engage with clients and their families to create personalized care plans that reflect their goals, preferences, and cultural values.
  • Provide counseling and emotional support during significant life transitions, helping clients navigate challenges with empathy and compassion.
  • Act as an advocate for clients, ensuring they have access to necessary resources and services that meet their needs.
  • Collaborate with an interdisciplinary team of healthcare professionals to coordinate care and address any concerns or obstacles faced by clients.
  • Monitor client well-being through regular check-ins and home visits, providing timely support and intervention when required.
  • Facilitate family communication to resolve conflicts and ensure everyone involved in the client's care is aligned and informed.
  • Educate clients and families about available community resources, support groups, and programs to improve their social engagement.
  • Maintain accurate and thorough documentation of assessments, care plans, and client interactions to ensure compliance with ethical and regulatory standards.
  • Stay updated on social work practices, community resources, and relevant policies to enhance service delivery and client outcomes.
Patient Population / Client Focus

In this role, you will primarily serve the senior population facing life-limiting illnesses and complex care needs, often in a hospice setting. Qualifications
  • Master's degree in Social Work from an accredited institution.
