Key Responsibilities
  • Conduct in-depth assessments of clients’ psychosocial needs, taking into consideration their emotional, mental, and social health.
  • Develop and implement personalized care plans that align with the unique goals and preferences of each senior client and their family.
  • Provide counseling sessions to seniors and their caregivers, guiding them through life transitions, challenges, and emotional struggles.
  • Act as an advocate for clients, facilitating access to essential community resources and services to ensure they receive holistic support.
  • Collaborate effectively with an interdisciplinary team that includes nurses, healthcare providers, and caregivers to ensure coordinated care.
  • Perform regular check-ins and home visits to monitor clients’ well-being, assess living conditions, and identify potential safety concerns.
  • Assist with conflict resolution among family members to promote harmony in the care provided to clients.
  • Educate clients and families on resource availability, support groups, and community programs that encourage social engagement.
  • Maintain accurate and timely documentation of all client interactions, assessments, and progress, ensuring compliance with ethical standards.
  • Stay updated on current social work practices, policies, and regulations impacting the field of gerontology.
Patient Population / Client Focus

The primary focus of this role is the senior population, many of whom may be experiencing complex emotional and social challenges related to aging. You will work closely with seniors and their families, understanding their unique circumstances and providing tailored solutions to enhance their quality of life.

Qualifications
  • A Master’s degree in Social Work (MSW) from an accredited institution.
  • Current and active state licensure as a Master Social Worker (LMSW).
  • A minimum of two years of clinical experience in social work, particularly within a community center or geriatric healthcare setting.
  • Expertise in social work principles, aging-related issues, and familiarity with relevant community resources.
  • Strong assessment, counseling, and problem-solving skills.
  • Demonstrated ability to connect with seniors and their families through empathy and compassion.
  • Exceptional communication and interpersonal skills, fostering teamwork with other healthcare professionals.
  • Proficient in utilizing electronic health record (EHR) systems and related computer applications.
Preferred Experience
  • Experience with case management, particularly in the field of geriatrics.
  • Familiarity with local community services, resources, and support systems tailored to seniors.
  • Background in conducting workshops or educational sessions focused on senior health and well-being.
Compensation and Benefits
