This position operates within a flexible remote environment, allowing you to work from the comfort of your home while still making a valuable impact in the lives of our hospice clients. While the role is primarily remote, regular video conferences and virtual meetings will be conducted to foster team collaboration and ensure seamless communication. You will receive robust support from our administrative staff to help you focus on providing the highest quality of care.

• Conduct in-depth psychotherapy sessions with patients, utilizing advanced therapeutic techniques tailored to individual needs.
• Employ a variety of psychological assessments and tests to accurately diagnose mental health conditions and identify appropriate interventions.
• Develop personalized treatment plans that outline clear goals and strategies to address the unique circumstances and preferences of each client.
• Facilitate crisis intervention services, including conducting suicide risk assessments and coordinating necessary hospitalizations when required.
• Work collaboratively with a multidisciplinary team including medical doctors, nurses, and other mental health professionals to provide integrated care.
• Maintain detailed and timely documentation of client interactions, treatment progress, and services rendered, ensuring compliance with regulatory standards.
• Contribute to enhancing clinical policies and procedures that promote effective mental health practices within our hospice care model.
• Participate in team meetings and ongoing training sessions to foster professional development and stay updated with best practices in social work.

The client focus of this role includes a diverse range of individuals dealing with terminal illnesses, their families, and caregivers. You will provide valuable support to those facing chronic pain, grief, and anxiety, ensuring culturally sensitive and empathetic treatment approaches that honor their unique experiences and backgrounds.

• Master’s degree in Social Work from an accredited institution.
• Current LCSW licensure to practice in the state of New Jersey.
• Minimum of two years of clinical experience working in mental health or social work settings, preferably with hospice or palliative care populations.
• Strong understanding of evidence-based therapeutic modalities including cognitive-behavioral therapy (CBT), dialectical behavior therapy (DBT), and mindfulness-based interventions.
• Demonstrated competency in addressing the psychological needs of diverse populations, particularly in grief and loss contexts.
• Exceptional written and verbal communication skills, allowing for effective interaction with patients, families, and healthcare teams.
• Ability to think critically and solve problems rapidly in high-pressure situations while displaying compassion and understanding.

• Experience working with hospice or end-of-life care environments will be prioritized.
• Familiarity with telehealth platforms and electronic health record systems to enhance service delivery.
• Training in family systems therapy or trauma-informed care is a plus.
