• Conduct thorough clinical assessments of patients’ mental health, establishing a comprehensive understanding of their emotional and psychological needs.
• Develop and implement individualized treatment plans that emphasize solution-based therapeutic interventions tailored to each patient’s unique circumstances.
• Engage in regular collaboration with a multidisciplinary treatment team, including physicians, nurses, and other healthcare professionals to coordinate patient care effectively.
• Maintain accurate, timely, and confidential clinical records in compliance with healthcare regulations while minimizing administrative burdens.
• Provide educational support and counseling to patients and their families, equipping them with tools and resources to cope with their emotional challenges.
• Stay informed about the latest mental health research, best practices, and effective therapies to continuously improve patient outcomes.

In this role, you will work closely with a diverse patient population, including individuals in hospice care who are facing terminal illnesses and their families. A significant focus will be placed on addressing not only the patients' immediate mental health needs but also the emotional well-being of their caregivers and loved ones.

• A Master's degree in Social Work from an accredited institution is required.
• Possession of a valid LCSW license in New York State is essential.
• Demonstrated knowledge of solution-based psychotherapy techniques and interventions suitable for hospice patients.
• Strong communication, interpersonal, and problem-solving skills, ensuring effective collaboration within a multidisciplinary team.

• Prior experience working in hospice settings or with terminally ill patients is highly desirable.
• Familiarity with electronic health record (EHR) systems and telehealth platforms is beneficial to streamline patient communication and record-keeping.
• Previous engagement with family counseling, grief support, or end-of-life planning is advantageous.
