• Conduct thorough clinical evaluations and comprehensive assessments of patients to develop individualized care plans that address their emotional, social, and spiritual needs.
• Collaborate with a multidisciplinary team, including physicians, nurses, and chaplains, to ensure seamless communication and delivery of well-rounded patient care.
• Facilitate family meetings to educate and empower families regarding care options, coping strategies, and available resources to support them through their loved one’s hospice journey.
• Utilize evidence-based practices to provide therapeutic interventions, such as individual counseling, family therapy, and crisis intervention tailored to the hospice patient population.
• Participate actively in weekly case reviews and contribute insights to optimize care plans, ensuring the highest quality of care is delivered consistently.

This role focuses on providing psychosocial support to individuals facing life-limiting illnesses. You will work primarily with elderly patients and their families, navigating complex emotions associated with grief, loss, and end-of-life decisions. Your expertise will be crucial in providing compassionate support, ensuring patients' dignity, and enhancing their comfort during this sensitive phase of life.

• A Bachelor’s Degree in Social Work (BSW) is required, with a preference for candidates holding a Master's Degree in Social Work (MSW).
• Current licensure as a Licensed Clinical Social Worker (LCSW) in New Jersey.
• At least 2 years of clinical experience in social work, preferably within hospice, palliative care, or related healthcare settings.
• Strong knowledge of clinical evaluations, care planning methodologies, and patient-centered practices.
• Exceptional communication skills to effectively collaborate with diverse stakeholders.

• Experience working with hospice patients and their families, navigating end-of-life discussions and emotional support.
• Familiarity with electronic health record (EHR) systems and digital communication platforms tailored for healthcare services.
• Training in grief counseling or related therapeutic techniques.
