• Conduct comprehensive clinical assessments to understand the unique needs of each patient and their family.
• Create individualized treatment plans based on best practices and evidence-based interventions tailored to each client's situation.
• Utilize therapeutic techniques to provide emotional support and guidance to patients and their families during this critical time.
• Maintain detailed clinical documentation and records that comply with all regulatory and organizational standards.
• Collaborate effectively with a multidisciplinary team to ensure a higher standard of coordinated care.
• Participate in case consultations and peer review meetings aimed at improving clinical outcomes and maintaining high-quality care.
• Educate and empower patients and their families by providing mental health resources and knowledge to help them cope with the emotional challenges presented by end-of-life care.

In this role, you will primarily serve patients dealing with terminal illnesses, many of whom experience a variety of emotional, psychological, and social challenges at this stage of life. You will support patients and their families through grief, anxiety, and the complex emotions associated with end-of-life decisions, helping to create a peaceful and meaningful experience for all involved.

• A Master's degree in Social Work (MSW) from an accredited institution.
• A valid LCSW license in the state of New Jersey.
• Minimum of two years of clinical experience in social work, specifically within the hospice or healthcare fields.
• Proven proficiency in solution-focused and evidence-based therapeutic techniques.
• Strong interpersonal, communication, and problem-solving skills.
• Ability to work collaboratively within a multidisciplinary team framework.

• Experience working in a hospice or palliative care setting.
• Familiarity with electronic health record systems (EHR) for accurate documentation.
• Advanced knowledge of local resources and support systems related to end-of-life care.

The successful candidate can expect a competitive salary ranging from USD 300,000 to 500,000 per year, commensurate with experience and qualifications.
