MDS Coordinator RN

Fairview Skilled Nursing Home

Position Summary

Fairview Skilled Nursing Home is seeking an experienced MDS Coordinator RN to oversee the MDS/RAI process, ensure timely and accurate assessments, maintain regulatory compliance, and support quality resident care and reimbursement.

Responsibilities
  • Complete and coordinate MDS, CAAs, and care plans.

  • Ensure compliance with CMS, Medicare, Medicaid, PDPM, and OBRA regulations.

  • Monitor assessment schedules and submission deadlines.

  • Collaborate with the interdisciplinary team on resident assessments and care planning.

  • Review documentation for coding accuracy and reimbursement.

  • Monitor Quality Measures and assist with QAPI initiatives.

  • Support survey readiness and educate staff on MDS documentation.

Qualifications
  • Current New York State RN license.

  • MDS Coordinator experience in a skilled nursing facility preferred.

  • Knowledge of MDS 3.0, PDPM, Medicare, Medicaid, and CMS regulations.

  • Strong organizational, communication, and documentation skills.
