Job description

Job Summary:
The Medicare Wellness Registered Nurse will schedule and conduct Medicare Annual Wellness visits in Primary Care Offices. Provides professional nursing care to patients/families, and the community. This role is essential in enhancing patient outcomes through proactive preventive care management under the supervision of a licensed provider. RN to perform comprehensive health risk assessments, identify preventive care needs, and collaborate with primary care providers to develop individualized preventive care plans based on patient history and risk factors.
Minimum Qualifications:
NYS licensed RN (or eligible), Bachelor's degree in Nursing or higher, plus a minimum of 1 year of direct patient care experience in a Primary Care setting OR Associate's degree considered with 3+ years of Primary Care experience. Experience with electronic medical records (EMR) required. Knowledge of medical office procedures and standards of medical care. CPR-BLS certification (American Heart Association or American Red Cross) is required by the time of appointment.
Preferred Qualifications:
Experience with Medicare wellness visits and collaborating with a multi-disciplinary team. EPIC experience preferred.
