As a member of our Community Health Clinic, you will be supported by a multidisciplinary team that includes medical professionals, case managers, and fellow social workers. • Psychosocial Assessments: Conduct thorough and timely evaluations of patients’ psychosocial situations, utilizing your understanding of human behavior to identify factors that may impact treatment outcomes.
• Clinical Interventions: Provide supportive counseling to patients and families, linking them with appropriate community resources, facilitating family meetings, and assisting with complex decisions related to treatment options, such as hospice or DNR protocols.
• Patient Care Management: Manage a dynamic clinical caseload by identifying high-risk situations and coordinating ongoing care, collaborating closely with case managers to prepare for effective patient discharges.
• Legal and Ethical Compliance: Adhere to all legal and ethical standards during assessments and interventions, incorporating policies related to patient rights and safety, such as duty to warn and reporting suspected abuse.
• Documentation: Maintain accurate and comprehensive records as per organizational policies, including progress notes and interdisciplinary care plans.
• Staff Training: Conduct in-service training sessions for medical staff to improve understanding of psychosocial issues and enhance patient care strategies.
• Quality Improvement: Participate in initiatives aimed at improving patient care through the development and analysis of relevant indicators.
• Additional Duties: Engage in other assigned activities, which may include planning events for Social Work Month or developing support groups for cancer patients and their families.

This role primarily focuses on serving patients within a community health clinic, including those facing chronic illnesses, mental health challenges, and significant life changes. You will work extensively with individuals and families dealing with cancer diagnoses and other serious conditions, providing tailored support and advocacy to enhance their quality of life. The culturally diverse patient population in Riverside embodies a wide range of backgrounds and needs, allowing for an enriching and meaningful practice environment.

• A Master's degree in Social Work from an accredited institution.
• At least 2 years of experience working in a medical setting, with a minimum of 1 year specifically in a cancer treatment environment.
• Current California Licensed Clinical Social Worker (LCSW) certification.
• Proficiency in English, both verbal and written, demonstrating strong communication skills.
• A commitment to excellence in patient care and exceptional customer service.
• Analytical, problem-solving, and evaluation skills, with the ability to make informed decisions independently and collaboratively.

The annual salary for this position ranges from $120,000 to $130,000, commensurate with experience and qualifications.
