  • Facilitate discharge planning by collaborating with healthcare providers, insurance representatives, and community resources to ensure a seamless transition from care settings.
  • Provide education and counseling to patients and families pertaining to adaptation to illness and the management of life changes.

  • Contribute to community service initiatives through education, consultation, and engagement with local support organizations to enhance resources for cancer survivors.
  • Support the professional development of emerging social workers through mentorship and training opportunities in oncology social work.
Patient Population / Client Focus

The LCSW will primarily work with patients facing serious illness, particularly within hospice settings. This role requires sensitivity to the diverse challenges experienced by patients and their families, including emotional distress, decision-making about treatment options, and navigating end-of-life concerns. Your expertise will be vital in fostering resilience and hope among clients who are coping with significant life transitions.

Qualifications
  • A Bachelorโ€™s degree in Social Work is required; an LCSW license is mandatory.
  • Proven experience in clinical social work, specifically within healthcare or hospice settings, is highly valued.
  • Ability to conduct comprehensive psychosocial assessments and develop effective treatment plans.
  • Strong communication and interpersonal skills, with a focus on empathy and client-centered care.
  • Familiarity with electronic health records (EHR) and telehealth platforms is preferred.
Preferred Experience
  • Experience in oncology social work or supporting patients with chronic illnesses.
  • Background in crisis intervention and familiarity with various therapeutic modalities.
  • Previous involvement in community outreach or education programs related to patient care or cancer support services.
