Heartful Counseling Solutions is seeking a dedicated and compassionate Senior Licensed Clinical Social Worker (LCSW) to join our remote team based in Salem, Oregon. In this vital full-time position, you will be responsible for delivering high-quality mental health services to individuals in a detox center setting. You will work collaboratively with a multidisciplinary team to promote patient recovery and well-being while addressing psychosocial challenges.

Work Environment

This position operates in a fully remote setting, allowing you to work from the comfort of your home while managing your caseload effectively. Our flexible hours are structured to support patient needs, with standard operations occurring Monday through Friday, from 9 AM to 5 PM. As part of a supportive remote team, you will utilize digital tools and telehealth platforms to connect with patients and colleagues.

Key Responsibilities
  • Conduct comprehensive psychosocial assessments through both patient and family interviews to identify needs and establish treatment goals.
  • Document and track client progress meticulously based on objectives set in individualized treatment plans, ensuring updates are shared with the care team.
  • Implement effective crisis intervention strategies for clients at risk, working proactively to prevent the escalation of symptoms.
  • Analyze assessment data and collaborate with other care professionals to craft cohesive care plans that address the multifaceted challenges faced by clients.

Qualifications
