• Conduct thorough clinical interviews and evaluations to assess the social, emotional, and economic needs of patients and their families, identifying barriers to medical care and support.
• Create individualized treatment plans that integrate trauma-informed strategies to address each client’s unique circumstances and promote overall well-being.
• Monitor client situations and follow up regularly to ensure progress towards therapeutic goals, making adjustments to treatment plans as necessary.
• Research and provide referrals to community resources such as healthcare services, food assistance programs, and childcare, enhancing clients’ access to critical support systems.
• Respond promptly to crisis situations, including potential cases of child abuse and mental health emergencies, ensuring optimal care and safety for those involved.
• Identify underserved populations and advocate for their needs within the community, promoting access to mental health services through outreach initiatives.

This position primarily focuses on delivering mental health support to individuals dealing with terminal illness within a hospice care framework. You will have the opportunity to work with diverse clients, including veterans and their families, helping them navigate the emotional complexities associated with end-of-life care. Your expertise will be vital in providing not only mental health support but also in fostering resilience and promoting dignity during challenging times.

• Master’s degree in Social Work (MSW) from an accredited program.
• Possession of a valid Licensed Independent Clinical Social Worker (LICSW) license.
• A minimum of 2 years of experience providing mental health services, ideally within a health-related context, with specific experience working with veterans preferred.
• Familiarity with the Department of Mental Health’s billing and documentation requirements.
• Strong background in parent-child intervention strategies and comprehensive knowledge of child development, parenting, and issues related to child abuse and neglect.
• Exceptional verbal communication skills to effectively counsel and advise clients in a sensitive and supportive manner.

• Experience utilizing electronic health record (EHR) systems to document patient interactions and manage treatment plans efficiently.
• Knowledge of community resources and systems that support mental health service delivery and patient advocacy.
• Background in trauma-informed care practices, particularly with populations affected by substance use disorders.
