• Conduct detailed psychosocial assessments of seniors, considering their emotional, mental, and social needs.
• Develop personalized care plans in conjunction with clients and their families, setting achievable goals based on individual preferences and circumstances.
• Provide individual and group counseling for seniors, addressing issues such as grief, mental health challenges, and transitions in later life.
• Act as an advocate for clients, helping them access community resources, healthcare services, and support groups tailored to their needs.
• Engage in interdisciplinary collaboration with healthcare professionals to ensure integrated care approaches that enhance clients’ overall health.
• Regularly visit clients to assess their living conditions, monitor psychological well-being, and proactively address any safety issues or concerns.
• Facilitate discussions among family members regarding the care of senior clients, addressing conflicts and enhancing communication.
• Educate clients and families about various community resources and programs that can increase their engagement and improve their quality of life.
• Maintain up-to-date and accurate documentation of client interactions, assessments, and progress, ensuring compliance with ethical and regulatory standards.
• Engage in continuous professional development, staying informed about the latest social work practices and regulations.

Our primary client focus is on seniors who may be dealing with a variety of challenges including chronic illness, mental health issues, and other age-related concerns. Working within a community health clinic framework, you will provide essential support to seniors and their families, empowering them to navigate the complexities of aging in a supportive and understanding environment.

• A Master's degree in Social Work from an accredited university.
• Current, valid state licensure as a Licensed Master Social Worker (LMSW).
• A minimum of two years of social work experience, ideally within a community health or geriatric environment.
• Thorough understanding of aging-related issues, social work principles and community resource availability.
• Excellent interpersonal and communication skills, enabling effective collaboration with clients, families, and interdisciplinary teams.
• Strong assessment and problem-solving abilities to address the diverse needs of clients.

• Experience working with elderly populations in a clinical or community health setting.
• Familiarity with electronic health record (EHR) systems for maintaining accurate client records.
• Proven ability to work independently and manage a caseload effectively.

This position offers a competitive salary ranging from $60,000 to $100,000 annually, depending on experience and qualifications. In addition to salary, we provide a comprehensive benefits package which includes:
