• Conduct thorough assessments and, when necessary, diagnoses for each patient, utilizing tools such as the Diagnostic and Statistical Manual of Mental Disorders (DSM) to develop individualized care plans.
• Provide ongoing evaluations of patient progress, adjusting treatment strategies as needed to address individual mental health diagnoses effectively.
• Maintain detailed and accurate treatment records and clinical documentation using current healthcare technologies, adhering to regulatory requirements and departmental protocols.
• Collaboratively design comprehensive treatment plans with patients and their families, ensuring they reflect the patients' objectives and cultural considerations.
• Proactively identify areas of patient safety risk and symptom management, anticipating issues and implementing interventions that foster positive outcomes.
• Facilitate referrals and establish strong connections with other clinical providers and referrals to optimize patient care within the community.
• Serve as a mentor, offering guidance and educational opportunities to students and interns, contributing to their growth and understanding of clinical practices.
• Participate in peer reviews and clinical collaboration under supervision, sharing insights that enhance team development and succession planning.

This position will focus primarily on patients and families facing the emotional challenges associated with cancer diagnoses. You will provide critical support to improve psychological resilience and coping mechanisms, helping them navigate their treatment journey and maintain a satisfactory quality of life.

• A Master’s degree in Social Work from an accredited institution.
• Current, valid State License as a Licensed Independent Clinical Social Worker (LICSW).

• Experience with outpatient therapeutic practices, particularly with populations affected by chronic illness or oncology patients.
• Familiarity with evidence-based methodologies in mental health treatment.
• Proven ability to foster collaborative relationships with diverse teams and stakeholders.
