RN Case Manager | Remote | Contract
Walker Healthforce is seeking an RN Case Manager with 3+ years of clinical nursing experience to provide comprehensive care coordination and case management services for health plan members. This is a Contract opportunity.
Start Date: ASAP
Location: Remote local to MI
Hours/Schedule: Monday-Friday, 40 hours
Worker Type: W2
Core Requirements
  • 3+ years of clinical nursing experience in acute, post-acute, or community care.
  • Current, active, unrestricted Michigan RN license.
  • Strong care coordination, assessment, and communication skills.
  • Ability to manage members telephonically and through digital communication platforms.
We Consider It A Bonus If You Also Have
  • Bachelor's Degree in Nursing.
  • Case management experience in a managed care environment.
  • Certified Case Manager (CCM).
  • Certified Chronic Care Professional (CCP).
  • Experience with telephonic and digital case management.
Job Responsibilities
  • Coordinate care across multidisciplinary teams.
  • Develop and manage individualized care plans.
  • Assess member needs and identify care gaps.
