Job description

About Matter Health

Matter Health is transforming healthcare by bringing primary and preventive care directly into affordable housing communities where older adults live.

Rather than asking patients to navigate traditional healthcare systems, we meet them where they areโ€”providing relationship-based care that improves access, strengthens communities, and helps older adults live healthier, more independent lives.

Our interdisciplinary teams partner closely with residents, property managers, and community organizations to create trusted relationships that improve health outcomes while making healthcare more personal and accessible.

As we continue expanding throughout Massachusetts, we're looking for passionate relationship builders who thrive in the community and want to make a meaningful impact every day.

About the Role

Matter Health is seeking a Property Success Manager to serve as the primary relationship owner for our affordable housing partners throughout Massachusetts.

This is a highly visible, field-based position responsible for developing, onboarding, and maintaining strong partnerships with property management teams while ensuring Matter Health delivers an exceptional experience for both residents and community partners.

This role is ideal for someone with an account management, customer success, community engagement, hospitality, or property management background who enjoys building relationships, solving problems, hosting events, and serving as a trusted partner.

Unlike a traditional sales role, success is measured by the strength of the relationships you build, the trust you create, and the experience you provideโ€”not by sales quotas.

What You'll DoBuild Strong Property Partnerships
  • Serve as the primary relationship owner for assigned affordable housing communities.

  • Develop trusted partnerships with property managers, service coordinators, and community leaders.

  • Become the first point of contact for property-related questions and concerns.

  • Build long-term relationships that support Matter Health's continued growth within each community.

Lead Property Onboarding & Launches
  • Coordinate onboarding activities for newly assigned properties.

  • Introduce property teams to Matter Health's care model and partnership approach.

  • Plan and execute successful property launches.

  • Ensure every new community receives a consistent, welcoming onboarding experience.

Strengthen Community Engagement
  • Host resident events that increase awareness and participation.

  • Represent Matter Health at community meetings and property events.

  • Build excitement around Matter Health services.

  • Ensure residents understand the value of relationship-based healthcare.

Protect the Matter Health Brand
  • Ensure Matter Health is represented professionally within every community.

  • Maintain strong communication with property leadership.

  • Monitor property satisfaction and proactively address concerns.

  • Maintain marketing materials and community presence.

Collaborate Across Teams
  • Partner closely with Engagement Specialists, Market Operations, Clinical Leadership, and Corporate Real Estate teams.

  • Share feedback from communities to improve operations and resident experience.

  • Help identify opportunities to strengthen partnerships and community engagement.

Successful candidates often come from backgrounds such as:

  • Community Engagement

You may be a great fit if you:

  • Communicate confidently with diverse groups.

  • Enjoy hosting meetings, presentations, and community events.

  • Are organized and comfortable managing multiple priorities.

  • Take initiative and solve problems independently.

  • Thrive in fast-paced, growing organizations.

  • Enjoy working in the field rather than behind a desk.

  • Adapt easily to changing situations and community needs.

Preferred Qualifications
  • Bachelor's degree or equivalent experience.

  • Experience working in relationship-focused roles.

  • Experience supporting community organizations, property management, healthcare, hospitality, or customer success.

  • Excellent communication and presentation skills.

  • Strong organizational and time management skills.

  • Valid driver's license and reliable transportation.

  • Ability to travel regularly throughout the assigned market.

Why Matter Health?

At Matter Health, every relationship matters.

You'll have the opportunity to directly impact the lives of older adults while helping strengthen the communities we serve.

This is an opportunity to build something meaningfulโ€”not simply manage accounts.
