Key Responsibilities
  • Conduct culturally sensitive and strength-based assessments for new patients and their families, gathering crucial information to inform personalized care plans.
  • Provide individual and family therapy to patients, tailoring therapeutic approaches based on identified needs and treatment objectives.
  • Efficiently complete billing procedures, documentation, individualized treatment plans, and insurance authorization forms within established time frames.
  • Develop comprehensive crisis plans, safety plans, relapse prevention strategies, and wellness plans to support clients in maintaining stability in their lives.
  • Create personalized discharge plans that outline follow-up care and support mechanisms for ongoing patient success.
  • Adhere strictly to federal and state regulations, as well as Centerpointe's policies and procedures, to ensure compliance in all aspects of care delivery.
  • Engage in ongoing professional development through training and continuing education, keeping your skills current and effective.
  • Participate actively in case consultations and agency meetings, contributing insights and collaborative strategies to enhance patient outcomes.
  • Complete all necessary licensure and mandatory training as required to maintain professional credentials.
Patient Population / Client Focus

You will work with a diverse population encompassing a variety of mental health challenges, including anxiety, depression, trauma-related issues, and more. Our commitment to culturally sensitive care allows you to build trust and rapport with patients from all walks of life, ensuring they feel understood and valued throughout their therapeutic journey.

Qualifications
  • A masterโ€™s degree in social work, counseling, marriage and family therapy, or a related discipline that meets licensure requirements.
  • Licensure as a Mental Health Therapist in the State of California, such as LPCC, LMFT, or LCSW.
  • Strong interpersonal and communication skills, with the ability to engage empathetically with clients and their families.
  • Experience with electronic health record systems and a willingness to adapt to new technologies.
Preferred Experience
  • Experience in outpatient mental health settings, particularly in a remote capacity.
