• Conduct in-depth assessments of clients’ social, emotional, and environmental needs to guide treatment and intervention strategies.
• Develop comprehensive service plans tailored to the unique goals and challenges of each client, ensuring regular progress evaluations.
• Facilitate individual and family counseling sessions, utilizing evidence-based approaches that foster personal growth and resilience.
• Engage in case management activities, including the coordination of resources, advocacy, and support to promote client autonomy and stability.
• Provide immediate crisis intervention, performing risk assessments, and safety planning when clients face acute challenges.
• Build and maintain collaborative relationships with community agencies to optimize service delivery and advocate effectively on behalf of clients.
• Participate in outreach initiatives designed to raise awareness of agency services, actively contributing to community engagement.
• Attend regular case conferences, team meetings, and ongoing training to stay current with best practices in social work and community health.

You will primarily serve individuals and families experiencing high levels of stress and adversity due to social determinants of health. Our clientele includes victims of domestic violence, individuals struggling with substance use, and people facing homelessness and economic instability. Through targeted interventions and community resource coordination, you will help these populations reclaim their power and improve their quality of life.

• A Master's degree in Social Work from an accredited institution is required.
• Possession of an active state license as a Licensed Clinical Social Worker (LCSW) is a must.
• Proven knowledge of social justice issues and community resources that support client recovery and well-being.
• Possess strong communication skills, cultural competence, and the ability to navigate complex social situations.

• Previous experience in a social service agency or similar environment will be an advantage.
• Familiarity with trauma-informed care principles and practices.
• Experience implementing case management systems and community outreach programs.

The salary for this position ranges from $60,000 to $100,000 annually, commensurate with experience and qualifications.
