Job Summary
The Social Worker – BSW provides supportive services to patients and families to promote psychosocial well-being, care coordination, and access to appropriate resources. Working under the supervision of licensed clinical Social Worker, this role assists with psychosocial assessments, discharge planning, referrals to community services, and education for patients and families. The Social Worker collaborates with the care team to ensure safe transitions and improved patient outcomes across the care continuum.
Essential Functions

  • Conducts psychosocial screenings for patients across the age spectrum, including neonatal, pediatric, adolescent, adult, and geriatric populations.
  • Identifies emotional, social, and financial needs of patients and families and assists in providing appropriate support or community referrals.
  • Supports the development and implementation of discharge plans in collaboration with licensed social workers and the case management team.
  • Coordinates referrals for services such as home health, long-term care, rehabilitation, or behavioral health resources as directed.
  • Assists in follow-up planning and facilitates communication with external providers, support agencies, and outpatient services.
  • Provides emotional support to patients and families facing illness, hospitalization, or complex discharge needs.
  • Participates in daily interdisciplinary care team discussions and supports case review and transition planning.
  • Maintains accurate, timely documentation in the medical record in accordance with organizational policies and regulatory requirements.
  • Maintains knowledge of community resources and helps connect patients to financial assistance, transportation, counseling, and support services.
  • Performs other duties as assigned.
  • Maintains regular and reliable attendance.
  • Complies with all policies and standards.

Qualifications

  • Bachelor's Degree in Social Work (BSW) required
  • 1-2 years of experience in a healthcare or social services setting preferred

Knowledge, Skills and Abilities

  • Knowledge of social work principles, basic case coordination, and psychosocial support practices.
  • Familiarity with healthcare systems and community-based resources.
  • Strong verbal and written communication skills.
  • Ability to collaborate effectively with clinical staff, patients, and families.
  • Professionalism, emotional maturity, and ability to maintain confidentiality.
  • Skill in managing multiple priorities in a fast-paced environment.
  • Basic proficiency with documentation systems and electronic medical records.

Licenses and Certifications

  • LCSW- License Clinical Social Worker if working in Behavioral Health required
  • Licensed Baccalaureate Social Worker (LBSW) if working in Care Management required
  • BLS - Basic Life Support obtained within 7 days of employment required
