Work Environment

This remote position offers a flexible and supportive work environment enabling work from home, with occasional onsite requirements at our Hospice Center in Brookhaven, NY, dependent on client needs. Key Responsibilities

• Conduct in-depth assessments to identify and understand clients’ unique social, emotional, and environmental challenges, allowing for tailored interventions.
• Develop comprehensive service plans that outline personalized goals, interventions, and strategies for clients and their families.
• Provide one-on-one counseling and emotional support for individuals and families coping with terminal illness, emphasizing coping strategies and emotional resilience.
• Work collaboratively with clients to coordinate service plans aligning them with community resources such as healthcare services, support groups, and financial aid programs.
• Offer crisis intervention, risk assessments, and safety planning to manage immediate concerns and crises effectively.
• Engage with various community agencies and organizations to facilitate resources and advocate on behalf of clients, ensuring their needs are met.
• Participate in community outreach initiatives to elevate awareness of hospice services and the importance of palliative care.
• Attend team meetings, case conferences, and ongoing training sessions to further develop skills and share knowledge with peers.

Patient Population / Client Focus

In this role, you will primarily provide services to individuals facing end-of-life issues and their families, fostering an environment of emotional support and dignity. Your efforts will focus on understanding their needs through compassionate communication and establishing trust, which is critical for effective counseling and support during this sensitive time.

Qualifications

• A Master's degree in Social Work (MSW) from an accredited institution is required, reflecting a strong foundational knowledge of social work principles.
• Possession of an active and valid state licensure as a Licensed Clinical Social Worker (LCSW).
• Demonstrated knowledge of trauma-informed care practices and a firm understanding of social and economic justice issues relevant to vulnerable populations.
• Exceptional communication skills with the ability to engage with diverse populations and build rapport quickly.
• Strong problem-solving capabilities, allowing for flexibility and adaptability in crisis situations.

Preferred Experience

• Experience working within a social service agency or healthcare setting, particularly with hospice or palliative care, is highly preferred.
• Familiarity with local community resources, service referral processes, and advocacy frameworks to assist clients effectively.
• Skills in conducting thorough assessments and planning tailored interventions aligned with client needs.

Compensation and Benefits

The salary for this position is competitive, ranging from USD 60,000 to 100,000 annually, complemented by a comprehensive benefits package.
