Join Our Team as an RN Care Manager

Bridgeway Health & Rehabilitation is seeking an experienced, compassionate, and organized Registered Nurse (RN) to join our team as an RN Care Manager. This is an excellent opportunity for an RN who enjoys coordinating resident care, collaborating with an interdisciplinary team, and making a meaningful impact on clinical outcomes and the resident experience.

As the RN Care Manager, you will serve as a vital link between nursing, therapy, physicians, admissions, social services, insurance providers, and our Central Authorization Team to ensure residents receive the highest quality care while supporting timely authorizations, efficient care coordination, and successful discharge planning.

Essential Responsibilities
  • Coordinate resident care from admission through discharge.
  • Manage Medicare, Medicare Advantage, Medicaid, and managed care authorizations.
  • Complete concurrent reviews and communicate with insurance providers to obtain continued stay approvals.
  • Collaborate with physicians, nursing, therapy, admissions, social services, and the interdisciplinary team to coordinate resident care.
  • Monitor resident progress, length of stay, and discharge planning to achieve positive clinical outcomes.
  • Identify barriers to discharge and develop solutions that support safe, timely transitions.
  • Ensure clinical documentation supports medical necessity and payer requirements.
  • Partner with the Central Authorization Team to prevent authorization lapses and reduce denials.
  • Participate in interdisciplinary care plan meetings.
  • Assist with payer communications, denials, and appeals as needed.
  • Educate residents and families regarding care plans, insurance coverage, and discharge planning.
  • Maintain compliance with all federal, state, and facility regulations.
Qualifications
  • Current Alabama Registered Nurse (RN) license in good standing.
  • Minimum of two years of nursing experience required.
  • Skilled nursing or short-term rehabilitation experience preferred.
  • Experience with case management, care coordination, utilization review, managed care, or insurance authorizations strongly preferred.
  • Working knowledge of Medicare, Medicare Advantage, Medicaid, and managed care guidelines.
  • Strong clinical assessment, critical thinking, communication, and organizational skills.
  • Ability to manage multiple priorities while maintaining exceptional resident-centered care.
  • Proficiency with electronic medical records.
