Job description

JOB DESCRIPTION

Mosaic Wellness & Recovery is seeking a compassionate, organized, and experienced Case Manager to support adults receiving substance use detoxification and residential mental health treatment at our Johns Creek facility.

This full-time, on-site role works closely with our clinical, medical, and residential teams to coordinate individualized care, address barriers to treatment, and connect clients with the services and resources needed for continued recovery and stability.

Key Responsibilities
  • Conduct comprehensive biopsychosocial assessments upon admission, including a review of each client’s medical, behavioral health, substance use, and psychosocial history.
  • Collaborate with clinical and medical team members to identify client needs and support appropriate treatment planning.
  • Develop and coordinate individualized, evidence-based care plans in partnership with clinicians and other members of the treatment team.
  • Coordinate referrals, transportation, applications, discharge planning, continuing-care services, and community resources.
  • Advocate for clients and help remove barriers to accessing appropriate treatment and support services.
  • Facilitate therapeutic and psychoeducational groups addressing mental health management, substance use recovery, coping skills, relapse prevention, wellness, and related topics.
  • Maintain accurate, timely, and compliant assessments, care plans, progress notes, group notes, and case-management records.
  • Communicate professionally with clients, families, referral sources, and multidisciplinary team members.
  • Respond appropriately to crisis situations and use effective de-escalation techniques.
  • Participate in staff meetings, required training, and continuing education.
  • Maintain professional boundaries and protect client confidentiality in accordance with HIPAA, 42 CFR Part 2, company policies, and applicable regulatory requirements.
Qualifications
  • Associate or bachelor’s degree in social work, psychology, counseling, human services, or a related field preferred.
  • Current Georgia substance use disorder treatment certification, such as CADC or CAC, required.
  • Previous case-management experience in behavioral health, substance use treatment, detoxification, or residential care required.
  • CPR and de-escalation certification required; facility-provided training is available if needed.
  • Knowledge of behavioral health services, community resources, and recovery support systems.
  • Strong written and verbal communication skills.
  • Excellent organization, documentation, and time-management abilities.
  • Understanding of trauma-informed and culturally responsive care.
  • Ability to respond to crisis situations calmly, professionally, and ethically.
  • Ability to collaborate effectively with clinical, medical, and residential team members.
