Job description

LaSante Health Center is a community health center dedicated to delivering high-quality, compassionate healthcare to underserved communities. We focus on improving health outcomes through preventive care, patient engagement, quality initiatives, and innovative programs that address community needs.

Position Summary

LaSante Health Center is seeking a Clinical Quality Lead to drive quality improvement initiatives and support exceptional patient care. Reporting to the Assistant Medical Director and Director of Strategic Initiatives, this individual will lead quality programs, improve preventive care performance, strengthen patient outreach efforts, support compliance and accreditation activities, and build systems that improve patient outcomes and organizational growth.

Key Responsibilities

* Lead organization-wide quality improvement initiatives across clinical programs and service lines

* Monitor and improve quality metrics including preventive screenings, pediatric programs, well visits, medication adherence initiatives, and payer incentive measures

* Develop patient engagement and outreach strategies to improve access, retention, and health outcomes

* Analyze data, identify gaps in care, and implement operational improvements to strengthen quality performance and compliance

* Support quality accreditations, regulatory readiness, PCMH initiatives, and value-based care programs

* Develop staff training initiatives to maintain quality standards and regulatory compliance

* Partner with clinical and operational leadership to build scalable systems that support growth

* Improve patient experience efforts, complaint resolution processes, and root cause improvement activities

* Prepare quality reports and performance updates for leadership

Qualifications

* Bachelor’s degree required; healthcare-related advanced degree preferred

* Experience in healthcare quality improvement, population health, clinical operations, or care management

* Knowledge of quality metrics, regulatory requirements, accreditation standards, and value-based care models

* Strong analytical, communication, project management, and leadership skills

* Community health center, FQHC, Medicaid, primary care, or behavioral health experience preferred
