A Program of Comprehensive Youth Services, Inc.
Position: Full time Case Manager
Qualifications: The Full time Case Manager must have a bachelors degree with a
major in criminal justice, psychology, social work or guidance and counseling, or
actively pursuing the above mentioned degrees. Must have a valid Drivers License
a clean driving record, must pass all required MDHHS background checks.
Experience: Experience and knowledge of youth development, experience in advocacy
and case management and experience in working with youth is desired.
Job Duties: The position requires a positive youth development and trauma informed
care approach, complete or assist in the completion of the Case Manager and Outreach
duties, as assigned.
Assist in the completing Intake/assessments, case management, and required
paperwork in a timely manner
Overseeing the completion of the client Educational referrals
Answering the agency crisis and information/referral calls
Participate in agency Intrastaff and monthly staff meetings
Provide individual and family Case Management and assistance in accessing
Community resources and housing assistance
Complete Client Care Monitoring for agency files minimally once a month
Oversee the completion of aftercare and follow-ups with exited clients
Follow all required local, state, and federal licensing requirements
Assist in the implementation of the programs structure
Compiles data and narrative descriptions for quarterly, semi-annual and
annual reports as assigned.
Assist with RHYMIS and HMIS data entry and reporting
Complete all required annual training hours
Participate in Community Coalitions and Networks as applicable
Other duties as assigned by the Program Manager
Hours: To be determined by the Program Manager or Director of RHY based on youth
and program needs, availability for some weekends and some holidays as needed
