Job description

You became a nurse to help people through some of the hardest moments of their lives.
Sometimes that happens at the bedside.
Sometimes it happens by helping a patient navigate a difficult diagnosis, coordinating the next step in their care, removing barriers that stand in their way, or simply taking the time to listen when they need someone most.
That's exactly what this Social Services Advocate & Case Manager role is about.
As an LPN or RN, you'll use your clinical knowledge in a different way. Instead of carrying a traditional patient assignment, you'll become the nurse who helps patients and families navigate the healthcare system from admission through discharge. You'll coordinate care, advocate for patients, remove barriers, connect people with community resources, and work alongside providers and nursing staff to improve outcomes across the hospital.
Our Mission: Quality, compassionate care for all.
Our Vision: Reimagine health care through connection, service and innovation.
Our Core Values: Be Kind | Trust and Be Trusted | Work Together | Strive for Excellence.
If you're a nurse who enjoys building relationships, solving problems, coordinating care, and making a lasting difference beyond a single shift, this Social Services Advocate & Case Manager position offers a unique opportunity to do exactly that.
Why This Nursing Role Is Different
This isn't bedside nursing.
It's also not a traditional case management position.
At Logan Health Chester, we've intentionally combined clinical nursing, patient advocacy, discharge planning, care coordination, and social services into one role because that's what best serves our patients and our community.
You'll continue using your nursing judgment every day while helping patients across observation, inpatient, skilled swing, intermediate swing, and outpatient services receive the right care, at the right time, with the right support.
As our hospital continues to grow, we've expanded this Social Services Advocate & Case Manager role to strengthen care coordination throughout the hospital. You'll help improve communication between patients, providers, insurance companies, community resources, and the care team while helping shape new processes for outpatient infusion services, referral management, discharge planning, and follow-up care.
You might be ready for this role if you've found yourself enjoying discharge planning, care coordination, patient education, or helping families solve problems just as much as direct bedside care.
What You'll Do
As our Social Services Advocate & Case Manager, you will:
  • Assess patients' clinical, psychosocial, financial, environmental, and discharge planning needs.
  • Identify barriers related to transportation, caregiver support, housing, insurance, behavioral health, legal concerns, end-of-life planning, and other social determinants of health.
  • Develop safe discharge plans and coordinate smooth transitions to the most appropriate level of care.
  • Coordinate referrals for swing bed, skilled care, rehabilitation, home health, hospice, behavioral health, durable medical equipment, and other community resources.
  • Serve as a trusted resource for patients, families, providers, and staff regarding insurance requirements, discharge options, and available services.
  • Help patients navigate Medicare, Medicaid, insurance requirements, prior authorizations, and community assistance programs.
  • Coordinate care planning, family conferences, utilization review activities, and interdisciplinary communication.
  • Complete psychosocial assessments, cognitive screenings, patient interviews, and documentation.
  • Advocate for patient rights while supporting informed decision-making and patient-centered care.
  • Help oversee abuse and neglect investigations while maintaining regulatory compliance.
  • Support outpatient infusion coordination by improving communication between providers, pharmacy, insurance, and patients so care is delivered without unnecessary delays.
  • Maintain timely, accurate documentation within the electronic medical record.
  • Identify opportunities to improve patient flow, care coordination, and the overall patient experience.

What Makes Someone Successful
The best Social Services Advocate & Case Manager isn't simply organized or clinically knowledgeable.
They're someone patients naturally trust.
You take time to listen. You understand that solving a problem often starts by hearing someone's story. Patients feel comfortable sharing difficult conversations with you because they know you're genuinely there to help.
You balance compassion with confidence. You can explain policies, regulations, and difficult situations with kindness while still advocating for what's right.
You're naturally curious and resourceful. If you don't immediately have an answer, you know how to find one. You build relationships throughout the organization and within the community because you know those connections ultimately help your patients.
You communicate comfortably with everyone, from patients and families to physicians, nurses, therapists, insurance representatives, and community agencies.
You know how to build trust quickly. Patients feel comfortable talking with you about their fears, frustrations, financial concerns, and personal challenges because they know you'll listen without judgment and help them find solutions.
You're firm when needed, especially when protecting patient rights and following regulations, but your approach is thoughtful, respectful, and compassionate.
Most importantly, you believe every patient deserves someone who will advocate for them.
If helping people through some of life's most challenging moments is what motivates you, you'll find purpose in this Social Services Advocate & Case Manager role.
Qualifications
Required
  • Current Montana RN or LPN license with authorization to practice nursing in Montana.
  • Minimum of two years of nursing or care coordination experience in a hospital, clinic, long-term care, or similar healthcare setting.
  • Comfortable with technology
  • RQI Provider certification within seven days of hire, transfer, or before providing independent patient care.

Preferred
  • Previous experience in case management, discharge planning, utilization review, or social services.
  • Experience working with community resources, insurance requirements, and post-acute services.
  • Case Management Certification.

The Skills You'll Bring
We're looking for someone who demonstrates:
  • Compassion, empathy, and integrity.
  • Critical thinking and sound clinical judgment.
  • Strong organizational and prioritization skills.
  • Excellent verbal and written communication.
  • The ability to work independently while collaborating across disciplines.
  • Professionalism when handling confidential and sensitive information.
  • A commitment to patient-centered care and high-quality outcomes.

The relationships you build with patients, families, providers, and community partners will directly improve how people experience healthcare during some of the most important moments of their lives.
