| Job Type: | Regular | Positions Supervised: No direct reports
Work Schedule: M-F 8-4pm, 37.50 weekly, flexible to meet program needs.
Under the supervision of the Executive Director, in collaboration with the Leadership team and Program staff, the Program Specialist I is responsible for providing, coordinating, and monitoring services for assigned participants, including the development, supervision and reviews of progress of an Individual Support Plan for each assigned participant assigned under Quest Inc. services.
Essential Functions Statement(s)
  • Coordinate and document the referral, intake, and discharge process for all new program participants.
  • Interview individuals, review records, conduct Initial assessment by the 60th day of service for newly enrolled participants.
  • Consult interdisciplinary professionals to evaluate the current skill levels, social skills and or competitive employment potential.
  • Collaborate in a positive manner with other staff counselors, physicians, and families to plan and coordinate services recommended and provided.
  • Develop (with the team) the Individual Support Plan. Monitor, evaluate, and record individual progress with respect to the Individual Support Plan.
  • Orient newly enrolled individuals to the program and staff at QUEST, Inc.
  • Counsel participants to assist them in dealing with any issues that are affecting their work at QUEST or living an everyday fife.
  • Educate participants and their families/community members about resources available to help support people with disabilities.
  • Work directly with the individuals in their program areas in order to understand their progress and assist them in moving forward with both hard and soft skills as well as developing interest areas for activities in the community.
  • Assist Individuals in adhering to plans, such as setting up appointments, arranging for transportation to appointments, and providing support.
  • Compile and document information pertaining to progress on the ISP. Assure all services are delivered as written in the ISP.
  • Refer Individual, or family to community resources for housing or other services to assist in their overall life satisfaction.
  • Meet regularly with Coordinators, Developmental Trainers, Employment Specialists, and Habilitation Specialists to review participants goals and progress.
  • Provide oversight of the 2380 Program area and the Older Adult Daily Living Center in collaboration with the Adult Day Program Coordinator.
  • Assist in scheduling and coverage of Small Group Employment, Supported Employment, Habilitation, Companion, 2380 Day Program and Older Adult Daily Living Center program.
  • Counsel and aid family members to assist them in understanding, dealing with, and supporting the individual.
  • Hold monthly, quarterly and annual ISP reviews and goal development within the time frames required by regulations and Provider Monitoring.
  • Provide assurance of agency compliance with all waiver standards under Chapter 51 Section 51.4.
  • Provide assurance and oversight of agency compliance with all Older Adult Daily Living Center as the administrator on-site:
    • Implementation of policies and procedures.
    • Admission and discharge of clients.
    • Safety and protection of clients.
    • Compliance with Chapter 11, Older Adult Daily Living Center.
  • Provide transportation for individuals we support as needed, either in a company vehicle or a personal vehicle. This could Include a meeting, volunteer opportunity, community event or emergency.
  • Serve as Initial Reporter, point person and Investigator when needed for EIM incident report system.
  • Other responsibilities or tasks as assigned by the Executive Director.

Requirements:
Education and Experience:
(1) A master's degree or above from an accredited college or university and 1 year of work experience working directly with persons with disabilities.
(2) A bachelor's degree from an accredited college or university and 2 years of work experience working directly with persons with disabilities.
(3) An associate's degree or 60 credit hours from an accredited college or university and 4 years of work experience working directly with persons with disabilities.
Computer Skills: | Basic Computer skills for Community Internet search, E-mail and Report writing.
Certifications & Licenses: | Current PA Driver's License- At least 3 years good driving record with valid vehicle insurance and inspection.
Clearances- PA Criminal check, Child Abuse, and FBI. Physical completion, as well as TB test.
Other Requirements: | Possession of a reliable vehicle for periodic transportation of program participants to varied work or volunteer sites. QUEST may also request staff to drive a company vehicle to transport participants to business sites for company tours, job exploration and recreational or volunteer opportunities in the community. New requirements - require people we support to be engaged in Community Activities at least 25% of their time in Program. Personal Assistance with a participant may be necessary on an individual basis.
