We are hiring a Program Coordinator for our Housing First Program! JOIN US at CMHA-YRSS; where innovation and community mental health care intersect! 

Closing Date: June 5, 2026

Position: Contract, full-time, 35 Hours per Week. Contract ending March 31, 2028. Evening Hours and Weekends may be required

Salary: $63,345- $76,704 per annum *Candidate experience will be considered when determining the starting salary

Location: The Program Coordinator will work from the Aurora Office located at 15150 Yonge St, Aurora ON L4G 1M2 on the days of on-site work. The incumbent will work out in the community/ client visits with regular travel in the York Region & South Simcoe communities as required. 

Job Profile:

The Program Coordinator will work in collaboration with the Manager to coordinate mental health, housing and financial support services at CMHA. The Program Coordinator will support the coordination of incoming client referrals, requests for immediate financial support, coordination with third parties including but not limited to private landlords, moving companies, insurance companies and cleaning services. As well, the Program Coordinator will support the flow of clients to mental health programming including groups and short-term case management services. The Program Coordinator will also provide direct mental health supports to housing clients and liaise with local community-based organizations to support program development.

Responsibilities:

Program Coordination:

  • Monitors incoming referrals for client services, community outreach and addresses requests for workshops and community events.
  • Assists with coordination of community events that are approved by the Manager.
  • For workshop and group requests, consults with the Manager to determine program capacity.
  • Receives and confirms receipt of new referrals and works in collaboration with referral sources to support prospective clients, conducts initial needs assessment screenings, enters and documents client activity in CRMS, and HIFIS and flows clients to the appropriate service(s) within the Housing Programs.
  • Monitors client waitlist. Collaborate with Clinical Supervisors in assigning clients to program staff as per the Manager’s directions and as per case load requirements and program needs.

Direct Service Delivery:

  • Deliver mental health and case management supports to a case load of clients.
  • Conduct needs assessments, housing supports, supportive counselling, referrals, case coordination, service navigation, and other individualized supports
  • Collaborates with other professionals to coordinate client treatment plans.
  • Documents all client-facing services in the CRMS system, HIFIS and manual tracker.
  • Monitor financial requests, liaising with Finance team, track spending and unique individuals served.
  • Maintains monthly statistical records of services provided and prepares a year-end summary of program activities as required.
  • Is familiar with and accesses a wide range of formal services and informal supports, as well as CMHA York Region services, to link clients to services, supports, and resources in the agency and larger community. Knowledge base will reflect York Region and South Simcoe.
  • Maintains current knowledge related to mental health and recovery.
  • Provides information to support reporting to funders.
  • In collaboration with the Manager and Director of Services, participates in the development of assessment and evaluation tools for the program as required.

Requirements:

  • A degree or diploma in health and social services, housing services, or related field.
  • Current registration with the OCSWSSW, OACYC, or other regulatory body under the Regulated Health Professions Act.
  • A second language (Cantonese, Mandarin, French, Farsi, Italian, Korean, Punjabi, Russian, Tamil, Urdu) reflecting the local community is an asset.
  • Four or more years experience in the mental health field. Experience within the housing sector is an asset.
  • Formal training in mental health, education, group work, program planning and evaluation, community facilitation, and development.
  • Knowledge/skills/experience in counselling, case management, and workshop/group development and delivery and familiarity with solution-focused brief therapy and/or other brief counselling approaches.
  • Ability to apply Recovery principles and empowerment-oriented philosophies and practices in work with consumers.
  • Ability to observe boundaries, engage in appropriate emotional regulation, refrain from dual relationships with clients, maintain confidentiality, and engage in reasonable self-care strategies designed to reduce stress by balancing work/life responsibilities.
  • Knowledge of, and adheres to relevant provincial laws, including, but not confined to the Mental Health Act, the Child and Family Services Act, the Health Care Consent Act, the Ontario Human Rights Code, Occupational Health and Safety Act.
  • Extensive knowledge of supports and services in York Region, including formal and informal resources.
  • Excellent diplomacy, decision making, problem solving, and teamwork skills.
  • A valid Canadian driver’s license, a minimum $1,000,000 third party liability insurance .
  • A satisfactory and current Vulnerable Sector Screening.
