Job Description
At University Health KC, we believe healthcare extends far beyond clinic walls. We are seeking a compassionate, resourceful, and community-driven Population Health Community Navigator to help bridge the gap between healthcare and the everyday realities our patients face.
In this role, you will empower individuals and families to overcome barriers to care, navigate complex healthcare systems, and access the resources they need to thrive. Whether supporting a patient through a Medicaid renewal, helping coordinate transportation to an appointment, or connecting someone to food, housing, or behavioral health resources, your work will directly impact lives across our community.
This is more than a healthcare role - it's an opportunity to become a trusted advocate, educator, and partner in improving health outcomes for vulnerable populations.
What You'll Do
As a Population Health Community Navigator, you will work alongside multidisciplinary care teams to engage high-risk patients and provide personalized support that improves both health and quality of life.
Key Responsibilities:
Patient Engagement & Care Coordination
  • Conduct Social Determinants of Health (SDOH) assessments and identify barriers to care
  • Engage high-risk patients through outreach, education, and relationship-building
  • Develop and support individualized care support plans
  • Assist patients with appointment scheduling, follow-up care, and referral coordination
  • Support smooth transitions of care following inpatient or emergency department visits

Chronic Disease Support
  • Coach and educate patients managing chronic conditions
  • Encourage medication adherence and care plan compliance
  • Help resolve transportation, communication, and access challenges
  • Document patient interactions and track progress toward health and wellness goals

Medicaid Navigation & Coverage Support
  • Guide patients through Medicaid enrollment and renewal processes
  • Help prevent gaps in healthcare coverage through proactive outreach
  • Connect patients with educational resources related to insurance changes and eligibility
  • Collaborate with Patient Access and clinical teams to ensure continuity of care

Community Advocacy & Resource Navigation
  • Connect patients with internal and community-based resources that improve stability and well-being
  • Advocate for patients and help strengthen patient-provider relationships
  • Support efforts to reduce appointment no-show rates and improve engagement

Team & Program Support
  • Serve as a mentor, trainer, and educator when needed
  • Support Community Health Workers and Population Health initiatives
  • Promote quality improvement, cultural diversity, patient safety, and organizational excellence
  • Represent the mission and values of University Health KC throughout the community

What You Bring:
Minimum Qualifications
  • Associate degree in a healthcare-related field or equivalent combination of education and experience
  • Certified Community Health Worker (CHW) or eligible to obtain certification
  • 3+ years of experience in a healthcare or community health environment
  • Knowledge of Medicaid/Medicare enrollment and coverage processes
  • Understanding of Social Determinants of Health (SDOH) and community-based care
  • Strong communication and relationship-building skills
  • Experience working with diverse populations and interdisciplinary teams
  • Comfort using laptops, tablets, and electronic health record systems

Preferred Qualifications
  • Missouri CHW Certification through Metropolitan Community College
  • Leadership or mentoring experience
  • Experience documenting within Electronic Health Records (EHR)
  • Knowledge of chronic disease management and community health impacts
  • Bilingual abilities are highly valued
