Community Health Worker

Established in 1978, Alcona Health Center (AHC) is a non-profit, Federally Qualified Health Center dedicated to making a lasting impact in the Northern MI communities we serve, offering medical, behavioral health, dental and pharmacy services. As a member of the AHC family, you will join a team of over 300 coworkers, all devoted to providing quality health care to all residents of our communities, regardless of their ability to pay.
Your Valued Contributions:
Provides outreach services in the community and in the clinics in an effort to improve patient care and outcomes.
  1. Registers and/or qualifies community members and clients for appropriate health center and community services; checks to ensure services are received and adjusts service arrangements as necessary.
    1. Convey the purposes and services of the program to patients and community partners.
    2. Conduct intake interviews with patients and provide referrals for services to community agencies as appropriate.
    3. Helps patients access resources including scheduling appointments, and assisting with completion of applications for programs for which they may be eligible
    4. Provides information to community members concerning AHC Health programs. Participates in outreach and conducts follow-up client health visits when necessary.
    5. Coach patients in effective management of their chronic health conditions and self-care
    6. Assist patient in understanding care plans and instructions
    7. Follow-up with health management/care plans with both patients and providers.
    8. Arrange or assist patients in connecting with transportation resources.
    9. Establish positive, supportive relationships with patients and provide feedback.
    10. Effectively work with people (staff, patients, providers, agencies, etc) from diverse backgrounds in reducing cultural and socio-economic barriers between clients and institutions.
  2. Communication and Health Information Entered into Record
    1. Enters appropriate treatment information into patient record in a timely fashion.
    2. Communicates patient’s health status to provider to insure adequate provision of care.
    3. Accurately documents activities on patient health record and other reporting forms.
    4. Progress notes entered into patient health record to insure appropriate ongoing care.
  3. Continuously expands knowledge and understanding of community resources, services and programs provided.
Required Training amp; Experience:
  • Use of Electronic Health Records
  • Word amp; Excel
  • Possesses a current State of Michigan driver’s license and valid automobile insurance.
