Summary Job Description:

The Case Planner Supervisor must be high energy with a bias for action. Must be able to effectively manage crises and conflicts. Practices and drives teamwork within and between program service units. Responsible for supervision and guidance to case planning team. Works across program departments to ensure resident goals are in progress and is committed to success as related to resident outcomes. Ensures documentation; reports, court hearings, meetings, trainings are scheduled/ attended as necessary for department expectations as set forth by agency and ACS/ OCFS and other stakeholders. Implements/ schedules training as needed and ensures case planning team compliance with agency procedure and protocol.

This position offers a full-time, 35 hour-per-week, field and in person work schedule and is based in Rockaway Park, New York. From time to time, this position requires work outside of normally scheduled working hours.

Our main campus in Rockaway Park, Queens, houses a Residential Treatment Center, Close to Home Non-Secure Placement programs, administrative offices, clinical services, and an on-site NYC Department of Education school. We also operate a Non-Secure Detention residence in Richmond Hill and Limited Secure Placement programs in Brooklyn andโ€ฏStaten Island.โ€ฏ

Principal Responsibilities

  • Demonstrates fidelity to the agencyโ€™s service values and organizational imperatives.
  • Monitors submission of progress notes, Court Reports, FASP, PHRโ€™s and other case planning documentation as required.
  • Conducts weekly supervision with Case Planners to ensure agency, ACS/ OCFS mandates and requirements are adhered to.
  • Responsible for review of documentation to ensure Case Plannerโ€™s are documenting casework and collateral contacts and case progress as it relates to ACS/ OCFS and agency mandates.
  • Assumes responsibility for case plannersโ€™ tasks/ actions as related to the established goals for assigned resident youth.
  • Provides 24-hour support to social service team and resident youth as it relates to permanency planning and contact resolution.
  • Attends Family Team Conference and/or other staffings and meetings as necessary/required.
  • Engages in bi-weekly group supervision with Director of Social Services and MST-FIT staff/program to monitor MST-FIT referrals made on behalf of residents/families.
  • Monitors and ensures time management and organization within social services to ensure adherence to the Qualified Residential Treatment Placement/other policies governing the department/ program.
  • Attends any/ all trainings as recommended/ required.
  • Assumes duties, as assigned, during the absence of the departments Program Director.
  • Any and all other duties as assigned.

Required Qualifications and Experience

  • MSW required with a minimum of 2 years of supervisory experience with some knowledge of child welfare, preferably in congregate care setting.
  • Mature, professional, leads by example,โ€ฏwith a strong sense of purpose and performance expectations/service excellence.
  • Fidelity to trauma informed care when engaging with resident population and staff alike.
  • Strong writing and communication abilities.
  • Proven supervisory success and basic child welfare knowledge.
  • Must be high energy with a bias for action.

Travel Requirements

This position offers a full-time, in-person and field schedule and is based in Rockaway Park and does require travel to other locations when needed.
