What You'll Do
  • Conduct inhome assessments and create individualized care plans
  • Provide clinical oversight, coaching, and support to caregivers and field nurses
  • Ensure compliance with LHCSA and NYS DOH standards through accurate, timely documentation

  • Support staff education and quality improvement initiatives
What You Bring
