Multiple Locations โ€“ Hillsborough, Pasco & Pinellas Counties

Job Summary

Pro Med Healthcare Services is hiring Per Diem Licensed Practical Nurses (LPNs) for hospice home care across Hillsborough, Pasco, and Pinellas Counties. This role offers flexible scheduling, day and night shifts, and the opportunity to provide compassionate end-of-life care in patientsโ€™ homes. Assignments are available in Town โ€˜nโ€™ Country, Lutz, Carrollwood, Brandon, Riverview, and surrounding communities. Candidates are matched to cases based on location and availability. Shift Options

  • Day Shift: 8:00 AM โ€“ 7:00 PM
  • Night Shift: 7:00 PM โ€“ 8:00 AM

What Youโ€™ll Do

  • Provide skilled nursing care to hospice patients in the home
  • Deliver patient-centered, comfort-focused end-of-life care
  • Follow individualized plans of care under RN supervision
  • Monitor patient condition and report changes or adverse reactions
  • Assist physicians and Registered Nurses with care delivery
  • Educate patients and families on medications, treatments, and comfort measures
  • Maintain accurate clinical documentation

Requirements

  • Active Florida LPN License (required)
  • Minimum 1 year of nursing experience (hospice, home health, acute, post-acute, or LTC preferred)
  • AHA BLS Certification
  • TB screening
  • Reliable transportation
  • Bilingual Spanish speaking โ€“ strongly preferred
