Job description

Job Title: Hospital Liaison – Respiratory & DME Services - Minnesota (Rochester)
Location (On-site, Remote, or Hybrid?): Rochester, MN (onsite)
Contract Duration: PERM
 

OVERVIEW
This role serves as the critical connection between hospital care and home healthcare services. You'll work closely with hospital teams to ensure patients receive the respiratory equipment and support they need for a safe and successful transition home.

  • Collaborate with hospital case managers, nurses, social workers, physicians, and discharge planners
  • Coordinate timely delivery and setup of respiratory and durable medical equipment
  • Educate patients and caregivers on proper equipment operation, maintenance, and safety
  • Complete insurance verification, referral documentation, delivery tickets, and patient records accurately
  • Support emergency equipment requests and troubleshoot issues as needed
  • Maintain strong relationships with hospital personnel and referral sources
  • Participate in quality improvement initiatives and maintain compliance with company and JCAHO standards
  • Provide in service education and CEU presentations when appropriate
  • Represent Rotech at professional events and community outreach opportunities
  • Travel to patient homes for equipment setup and follow up using a company vehicle
