What You’ll Do:  

  • Deliver personalized hospice care that honors each patient’s unique needs—document every step to ensure accuracy and continuity. 
  • Lead with empathy during home visits, assessing patient and family needs and crafting care strategies that prioritize comfort, dignity, and peace. 
  • Empower families and caregivers through clear education on hospice services, symptom management, and available support resources. 
  • Guide and mentor your team, training Hospice Aides and LPNs to deliver safe, effective, and compassionate care at home. 
  • Collaborate seamlessly with physicians, interdisciplinary teams, and facility staff to create smooth transitions and coordinated care plans. 
  • Provide hands-on expertise in pain control, medication administration, and symptom relief—bringing comfort when it matters most. 
  • Be a trusted resource after hours, participating in on-call rotations to ensure patients and families receive timely support on weekends and holidays. 
  • Maintain compliance and eligibility by monitoring patient status and completing all certification and recertification documentation with precision.
