Key Responsibilities
  • Client Admission: Collaborate with the team to admit clients needing medication administration and nursing services.
  • Assessments & Monitoring: Perform nursing assessments, monitor client progress, and coordinate care.
  • Task Delegation: Delegate nursing tasks to caregivers, ensuring compliance with regulations.
  • Care Plan Development: Lead the development and updating of individualized care plans.
  • Supervision: Provide guidance to LPNs and ensure high-quality care delivery.
  • Communication: Maintain clear and effective communication with the team and clients.
  • Documentation: Review client documentation and records, and document nursing services and interactions.
  • Training & Education: Deliver training and education to caregivers to enhance their skills.
  • Meetings: Participate in company and departmental meetings to stay informed and engaged.
  • Collaboration: Work collaboratively with fellow healthcare professionals to ensure comprehensive care.
  • Policy Adherence: Ensure compliance with agency policies and regulations.

Requirements
  • Licensure: Active Oregon state RN license.
  • Background Check: Ability to pass a state and national background check.
  • Transportation: Personal vehicle, valid driver's license, auto insurance, and a clean driving record.
  • Experience: 1 year of experience in a home care/home health setting OR at least 1 year in an SNF, ICU, or Med-Surg.
Work Schedule
  • Hours: Expected to work 10-30 hours per week based on business needs and your schedule.
  • Typical Office Hours: Monday - Friday, 8 AM - 5 PM.
  • Location: 60% of your time will be in an office environment; 40% will be in various client homes for assessments or observations.
