Job description

The Inpatient Hospice Licensed Practical Nurse (LPN) provides compassionate, patient-centered end-of-life care to patients admitted to the hospice inpatient unit. The LPN supports the interdisciplinary team by delivering direct patient care focused on comfort, dignity, and symptom relief, under the supervision of a Registered Nurse (RN).

Summary

The Licensed Practical Nurse has the responsibility and accountability for assuring delivery of quality nursing care for hospice and home care patients under the direction of a Registered Nurse in accordance with individual patient needs, physician orders, hospice and departmental policies. The LPN will provide hospice/palliative and home care services with respect for human dignity and the uniqueness of the patient, unrestricted by consideration of socio-economic status, personal attributes or the nature of the health problems. Maintains confidentiality of information regarding patient/caregiver, and all health care team members.

Education 

Must be a graduate of a school of nursing. Must complete hospital general orientation and department orientation (Hospice).

Licensure/Certification 

Must possess a current and valid license as a Licensed Practical Nurse from the Indiana State Board of Nursing. Must have valid state driver's license, reliable transportation, and proof of auto insurance. Must have current CPR certification.

Experience 

Must have two years of experience in home care, oncology, critical, med/surg or nursing home.

Other Qualifications

Must be able to lift 50 pounds and assist with 100 pounds.
