The Hospice Triage Registered Nurse (RN) receives incoming triage calls from patient and providers, addressing changes in physician orders and patient care needs as appropriate.

WHAT YOU CAN EXPECT

  • Position is in-person, located at the Hospice House.

  • Weekend Hours: Saturday and Sunday, 8am to 8:30pm.

  • Assists patients, families, clinical staff and providers via telephone with clinical issues, problems and questions.

  • Reports change in patient status to appropriate clinician.

  • Participates in case conferences and interdisciplinary team meetings and coordinates changes in physician orders with clinical staff and providers.

  • May provide nursing visits as required to deliver patient care according to home health policies.

  • Completes required documentation within the established timeframe.

  • Receives physician orders, providing follow-up with appropriate clinical staff; may include scheduling.

QUALIFICATIONS

  • Associate's Degree Accredited Registered Nurse Program- Required

  • Registered Nurse (RN)- State Licensing Board - Required

  • Cardiopulmonary Resuscitation (CPR) - Required

  • Auto Insurance - Required

  • Driver's License - Required

  • 1 year- Hospice Experience- Preferred
